A situation... PLEASE TELL ME WHAT YOU THINK!!!!!

HERE IS THE SITUATION TELL ME WHAT YOU THINK!!

You have had a bone biopsy and as a result you have gotten an infection in the incision and the start of an infection in your bone.

When you go to see your surgeon for an emergency consult you have been on Keflex 500 mg 4X /day for 11 days, and Clindomyacin 300 mg 3X/day for 6 days.  You had had to ER once because of pain from the incision and increased bleeding and green discharge.

The surgeon looks at your wound and tells your that you have to options:

1) The first option is the one he likes the least and that is to do surgery again that day. He says that if you do the surgery what will happen is this: they will make a slightly larger incision they will take tissue and bone samples to be sent to pathology and then they will close the wound if it isn't too infected. If it is overly infected they will leave it open for two days and then bring you in for another surgery to wash it out and close it, at that time they would then take the tissue and bone samples.  Either way you would spend atleast 4-5 days in the hospital after they collected the tissue and bone samples so that Pathology could analyze them and tell them what type of IV antibiotics you need to be on.  Then they would put in a PICC line and give you your first dose at the hospital. They would then arrange for you to get the rest of your IV antibiotics at home.  You would be out of school for atleast 2 weeks.  If when they close the incision it gets infected again or opens up again then they will leave it open and you will then have to pack it for 6 weeks. *

2) The second option is to continue with the antibiotics for another 6 days then have you call them and if  there is any syptoms still any of the bleeding, gooing of green discharge or still any pain, they will then do surgery because at that time you will be a month out from your surgery.  *

*Feel free to substitute work in for school if it makes it more real for you*

My question is what you would do? What would you choose?

Bri, I would wait.  Antibiotics are powerful.  Your Doctor doesn't sound like he wants to put you through another surgery.  Keep a very close eye on things.  Diana

Diana,

I tend to agree, but it actually wasn't my surgeon. The surgery was done by Dr. Yasemzksy and his resident Dr. Wendt from Mayo.  Dr. Yasemsky is currently  out of town, so we have been going through Dr. Wendt. When we went down there, Dr. Wendt looked at it and then brought in another colleauge of Dr. Yasemzksy's, Dr. Huddleson.  He introduced himself as "a spine dude", but he looked at it and he was the one who gave me my options he also told me that it was completely up to me & if I just felt like I was completely sick of taking meds and done with feeling crappy like I did then they would admit me to the hospital that day and schedule me for surgery the next.  We  are technically three more days into the week, and it hasn't improved, if anything it has gotten worse.  So we will see, if anything we were optomistic and its always better to be optomistic than pesimistic.

Thats a tough predicament ! If your having green discharge thats obvious a bad sign but being on those heavy duty antibiotics apparently are not helping >> I think your wound needs to be debreeded and re dressed. You might even have to go on IV antibiotics at a stronger dose before he does another surgery?? If you wait too long for another surgery, the infection might get worse and things could turn traagic

Bri, you have a heavy load for one so young.  I would do just what you are and wait on the surgery.  Because you are so close to finishing the semester, giving out a whole week would be difficult and possibly impossible to make up.  I wish you luck and I honor you and your mother for your continued faith and strength.  Bobbie

So time for an update!!! I ended up having emergency surgery yesterday.  They went in expecting to find an infection and to clean it out. Well when they got in there, there was no infection.... what they found was that my body litterally had not healed inside or out!! It looked on the inside as if they had just done the surgery the day before not three weeks before.... the only difference was that the dissolving stiches had dissolved and there was no evidence that they had ever been there. This time they cleaned it out.... and stitched it inside and out... hopefully this time it will heal.
