Hello,  You mentioned hyperpigmentation under the arms today and gave recommendations on how to lighten the area.  I am a primary care physician, with a background of producing and hosting medical talk shows. Physical signs and symptoms can indicate underlying medical problems, and I believe we have an obligation to mention these potential undiagnosed issues.  Acanthosis Nigricans, has the component of hyperpigmentation, and it can be suggestive of an underlying issue with glucose metabolism.  Even today, there are many who walk around with undiagnosed diabetes.  Please tell your audience about acanthosis nigricans. Also if hydroquinone is improperly used you can create patchy discoloration of the skin.  If a man presents with erectile dysfunction, I look for the etiology, and it is not uncommon to find that the diagnosis is associated with diabetes or high blood pressure.  If I see hyperppigmentation, in the axilla, I carefully check the texture, and do a fasting blood glucose.  I hope this is helpful. Dr J
